Enter your first & last name, date of birth and an email OR phone number.

Confirmation via email/SMS
Depending on whether you provided your email OR phone number, you will receive a confirmation code — enter it.

Speak with your doctor
Use the link from the second email ("Appointment confirmed") and click "Join".
